Several of our dentists are contracted with the PPO plans listed here. Our other dentists are fee-for-service providers and are not contracted with dental insurance companies — we’re happy to tell you which applies to the dentist you’d like to see.

We electronically submit claims and provide all the necessary documentation to expedite the process. A typical claim is paid within 30 days. We’ll always do our best to help you maximize your benefits.

We require payment at the time of service. Keeping our administrative costs down is how we avoid unnecessarily raising our fees.

Pay however is easiest for you — all major cards plus CareCredit are welcome at every office.

Many treatments are phased across multiple appointments. Our office manager can work out an arrangement with you.

New Patient Questions

Your completed new patient forms, a photo ID, your dental insurance card and a list of any medications you’re taking. If you’d like us to bill your insurance, bring the credit card you’d like kept on file.
Several of our dentists are contracted with MetLife, Cigna, United Healthcare, Aetna, Guardian, Delta Dental, Blue Cross Blue Shield and Humana. Others are fee-for-service providers. Call us and we’ll confirm what applies to the dentist you want to see.
Dental plans may exclude or discourage certain treatments due to policy limitations — that doesn’t mean the treatment is inappropriate. We’ll always explain your options and costs before you decide.
We submit claims electronically with all the supporting documentation. Most claims are paid within 30 days. If your insurer doesn’t pay within the agreed window, the balance is billed to the card on file and we send you all the claim paperwork.
VISA, Mastercard, Discover, American Express and CareCredit. Payment is due at the time of service.
Many treatments are phased across multiple appointments. Our office manager can work out a payment arrangement with you, and third-party financing is available on request.
We ask for at least 24 hours’ notice. Because we don’t double-schedule, a missed appointment or short notice carries a $75 per hour cancellation fee.
Fee estimates can be extended for 90 days from the date of your examination.
