Dr. Jack is a member of the American Academy of Cosmetic Dentistry (AACD), American Dental Association, and Illinois State Dental Society and Chicago Dental Society. He completes 100 hours of continuing education courses and conferences.

A native of Orland Park, Illinois, he enjoys painting and skiing in his free time. He and his wife Lana enjoy exploring the city’s vibrant restaurant scene and art museums and spending time with their Labradoodle.
