Dr. Jack earned a bachelor of Pre-Dentistry and Political Science from DePauw University in 1992 and a DDS degree from the University of Illinois in 1996. He opened his first practice in 1999, Lincoln Park Smiles. Now as LPS Dental, that practice has three offices across Chicago.
